Mensen met FASD begrijpen sociale relaties vaak slecht. Hun fysieke sexualiteit ontwikkelt zich echter normaal en ze komen vaak vroeg in de puberteit. Deze combinatie van sociaal zwak functioneren en normale fysieke sexualiteit leidt vaak tot tragische omstandigheden. Volgens een studie van Ann Streissguth vertoont 49% ongepast sexueel gedrag.
